<<Joint letter: Nobel Peace Laureate Urgently Needs Essential Medical
Care for Serious Health Problems
In a joint letter released today, the Center for Human Rights in Iran (CHRI)
and 43 human rights organizations in the Free Narges Coalition urgently
call on the UN Human Rights Council to intervene for the immediate
medical release of Iranian Nobel Peace Prize Laureate Narges Mohammadi
Narges Mohammadi, forced to endure over a decade in and out of prison on
baseless political charges, has repeatedly faced medical neglect. Most
recently, she was returned to Evin Prison just two days after undergoing
complex surgery to remove a suspected cancerous lesion in her leg-a
procedure involving a bone graft-against her doctor's advice and legal
requests for a medical furlough and sentence suspension to ensure proper
care.
"Prison authorities' withholding of essential medical treatment from
Mohammadi displays a callous disregard for her health and wellbeing,"
the coalition stated. As Iran undergoes its Universal Periodic Review,
the coalition is demanding that Iranian authorities grant Mohammadi a
medical furlough on humanitarian grounds and uphold the rights of all
unjustly detained human rights defenders. "We, the undersigned
organizations, are making an urgent call for her full and unconditional
release as she should not be in prison in the first place, and in the
interim, to be granted an immediate medical furlough on humanitarian
grounds, given the precarious state of her health and her need for
comprehensive care," the coalition added. CHRI strongly echoes the
coalition's call for Mohammadi's immediate and unconditional release and
urges the UN Human Rights Council to pressure Iranian authorities to
respect and protect the rights of all detainees.

Free Narges Coalition and Partners Urge UN Human Rights Council to
Intervene for Narges Mohammadi's Urgent Medical Release
To the United Nations Human Rights Council,
We, the undersigned free expression and human rights organizations,
write to you in response to the recent news that jailed Iranian Nobel
Peace Prize Laureate Narges Mohammadi is in urgent need of medical care.
Ahead of Iran's review under the UN Human Rights Council's Universal
Periodic Review (UPR) mechanism, which takes place over the next two
months in Geneva, we urge you to call on the Iranian authorities to
grant Mohammadi a medical furlough on humanitarian grounds so that she
is able to receive comprehensive and essential care for a range of
serious medical conditions.
Mohammadi, a woman human rights defender, journalist, author, and former
deputy director and spokesperson of the Defenders of Human Rights Centre
(DHRC) in Iran, has spent more than 10 years of her life in prison, with
her current period of detention starting in November 2021. She is
currently serving sentences totalling 13 years and nine months in
prison, on charges including committing <propaganda activity against the
state> and <collusion against state security.> An internationally
renowned writer and activist, she is the recipient of numerous
international awards for her tireless struggle for human rights,
including the 2023 Nobel Peace Prize, the 2023 UNESCO/Guillermo Cano
World Press Freedom Prize, the 2023 PEN/Barbey Freedom to Write Award,
and the 2022 Reporters Without Borders Prize for Courage. Mohammadi's
health has deteriorated drastically during her long incarceration, most
notably in 2022, when she suffered multiple heart attacks before
ultimately being transferred to hospital for emergency heart surgery. In
early October 2024, Mohammadi's family expressed serious concerns
regarding repeated refusals by Evin prison officials to transfer her to
a hospital for appointments to carry out an angiography, an intervention
that was prescribed by her cardiologist in March; she was finally
allowed to attend an appointment on October 27, 2024. On November 3, her
lawyer announced that during her recent medical visit, doctors
discovered a bone lesion in her right leg suspected of being cancerous.
Although Mohammadi underwent surgery to remove part of the bone in her
lower leg, including a bone graft, on November 14, she was transferred
back to prison after only two days, against her doctor's advice and
another request from her legal team that she be granted a medical
furlough and sentence suspension. Years of imprisonment and months of
solitary confinement have severely compromised Mohammadi's health,
leaving her with multiple serious conditions that cannot be addressed
through a short, incomplete hospital visit. Prison authorities'
withholding of essential urgent medical treatment from Mohammadi
displays a callous disregard for her health and wellbeing under
detention. Worryingly, her case is not unique, but is part of a
systematic pattern of arbitrary medical neglect of prisoners, including
human rights defenders, journalists, and writers. In a particularly
egregious example, poet and filmmaker Baktash Abtin died in state
custody in January 2022 after delays in being provided with timely
medical care. The news of Mohammadi's deteriorating medical condition
comes amid a current wave of denying medical care to multiple prisoners
of conscience in Iran, particularly less well-known detainees. We echo
the recent call from 22 prisoners in the women's ward of Evin Prison, in
which they hold the Iranian government and judiciary responsible for
creating the conditions whereby the lives of prisoners are put at grave
risk, and appeal to international human rights stakeholders in joining
them to push for change. As Mohammadi marks the third anniversary of her
unjust detention on November 16, we, the undersigned organizations, are
making an urgent call for her full and unconditional release as she
should not be in prison in the first place, and in the interim, to be
granted an immediate medical furlough on humanitarian grounds, given the
precarious state of her health and her need for comprehensive care. In
addition, we urge that Iranian authorities stop the criminalization of
human rights, and refrain from summoning human rights defenders,
journalists, and writers to serve their prison sentences while their
health situation is precarious. We urge the UN Human Rights Council to
ensure that the Iranian government commits to implementing without delay
a recommendation that it accepted during the last UPR cycle five years
ago, namely to ensure that "all individuals in custody receive adequate
health care and treatment, including preventive measures, such as
screening for medical conditions, free of charge and without
discrimination." Finally, we reiterate our broader call that all those
unjustly detained for their human rights work, including expression, be
immediately and unconditionally released, and urge the UN Human Rights
Council and national and international stakeholders to join us in
pressing the Iranian authorities to uphold the basic human rights of all
Iranians, especially those who are being prosecuted for their human
rights work.

Arezu Azarm-Sefat, a political prisoner held in Tehran's notorious Evin
Prison, has been suffering from severe shoulder and arm pain for some
three weeks. Despite her condition, she has been denied adequate medical
attention, raising serious concerns about her health and the prison's
negligence in providing essential healthcare.
Worsening Condition Without Proper Treatment
The condition of Arezu Azarm-Sefat has significantly restricted her
movement, leaving her unable to use her arm. Attempts to alleviate her
pain, such as immobilizing her arm with a sling, have proven
ineffective. Even the slightest contact with her arm intensifies her
pain. According to sources close to her family, Arezu was reluctantly
transferred to the prison's clinic only after protests by her fellow
inmates. However, she was sent back to her cell without undergoing
medical imaging or a thorough examination. The prison doctor reportedly
dismissed the need for urgent medical care, advising her family to seek
treatment outside the prison-an option not currently available to her.
This neglect contradicts the legal obligation of the Iranian regime to
provide diagnostic and treatment services for prisoners with health
issues.
Family Struggles and Systemic Neglect
Arezu Azarm-Sefat has an elderly mother who is unable to actively
advocate for her daughter's health, further compounding the difficulty
of securing proper care. The ongoing pain and lack of treatment have
heightened fears that her condition could worsen or lead to long-term
physical complications.
Background on Arezu Azarm-Sefat
Arezu Azarm-Sefatwas first arrested by Iranian security forces on May 1,
2024, and detained in Ward 209 of Evin Prison, a section run by the
Iranian Ministry of Intelligence. After completing her interrogation,
she was temporarily released on bail on May 20, 2024. Previously
convicted on charges of alleged collaboration with opposition groups,
she was initially sentenced to five years in prison. Following a legal
appeal, her sentence was reduced to three years and nine months. On
August 26, 2024, Arezu was taken back into custody to serve her reduced
sentence in the women's ward of Evin Prison.
A Systematic Denial of Rights
Arezu's case highlights the broader issue of the Iranian regime's
systemic neglect of prisoners, particularly those incarcerated on
political grounds. Denying prisoners access to medical care is a common
tactic used to exert psychological and physical pressure, further
exacerbating their suffering.
The ongoing negligence in providing medical care to Arezu Azarm-Sefat
exemplifies the dire conditions faced by political prisoners in Iran.
Her worsening health, coupled with the prison’s refusal to meet her
basic medical needs, underscores the Iranian regime's disregard for
human rights.
This situation calls for international attention and action. Advocacy
groups and global institutions must press the Iranian regime to uphold
its legal and moral obligations to ensure the safety and well-being of
prisoners under its custody. Arezu Azarm-Sefat's case is not just a
personal tragedy-it is a stark reminder of the ongoing human rights

<<Parvin Mirasan: Medical Care Denied to 67-Year-Old Prisoner
Parvin Mirasan, a 67-year-old political prisoner, is currently serving a
four-year sentence in Evin Prison. Despite needing specialized medical
attention, she was recently taken to a hospital but was returned to
prison shortly after, without completing the necessary treatment. Parvin
Mirasan suffers from multiple ailments, including diabetes, Parkinson's
disease, and joint issues, and she also occasionally experiences bowel
adhesions. She endures severe knee pain, with her left knee frequently
swelling, making it difficult for her to walk. Two weeks ago, after
months of being denied treatment, she was finally taken to the hospital,
where doctors advised that her left knee requires surgery. Nevertheless,
she was returned to prison without the necessary medical intervention,
only receiving pain relievers. Ms. Mirasan sometimes experiences severe
forgetfulness and prolonged silences, staring into space; at other
times, she speaks suddenly and at length. Despite her multiple illnesses
and neurological issues, she has still not been allowed a referral to
the medical examiner to assess her inability to withstand prison
conditions. Parvin Mirasan was arrested on November 4, 2022, during
nationwide protests by intelligence agents in Shiraz and was initially
held in Adelabad Prison. In February 2023, she was transferred to Tehran
and, on March 18, 2023, after her interrogation process ended, was moved
to the women's ward of Evin Prison. After 17 months of uncertainty, she
was sentenced in April this year by Branch 26 of the Tehran
Revolutionary Court to four years in prison on charges of assembly,
collusion, and propaganda against the state. Born in 1957, Parvin
Mirasan lost both of her sons when they were young and now has only one
daughter, who lives abroad.>>

<<Prisoner dies from lack of medical attention in Zabol, Iran
A Baloch prisoner named Noorali Kohkan died in Zabol Prison due to a
lack of adequate medical attention. According to a report received by
the Hengaw Organization for Human Rights, on Thursday, October 17, 2024,
40-year-old Noorali Kohkan, from Nimruz County, Sistan and Balouchistan
province, passed away in Zabol Prison after experiencing serious health
complications that went untreated by prison authorities. Rasad
Balochistan news outlet reported that Kohkan had been suffering from
heart problems and difficulty breathing. Despite his deteriorating
condition, the prison's health staff ignored his symptoms, only offering
him painkillers instead of providing the necessary medical treatment.
The family was notified of Noorali's worsening condition through a phone
call from another prisoner. When they arrived at the prison, they were
informed that Noorali had passed away and that his body was in the
morgue. In response, Noorali Kohkan's family has filed a complaint
against the authorities of Zabol Prison, demanding answers regarding his
death. However, they have not yet been granted access to his body.
Noorali Kohkan had been serving a 10-year sentence after being convicted
of participating in a kidnapping. He had been in prison for two years
prior to his death.>>

Crisis
Political prisoner, Sarvnaz Ahmadi, has been on a medication strike
since September 7, 2024, protesting the rejection of her medical
furlough request and the delay in delivering her essential medications.
Sarvnaz Ahmadi, who suffers from a neurological condition, recently
experienced another severe nervous attack. Despite her deteriorating
physical health, she has continued her medication strike. This strike
has caused her to suffer from persistent headaches, insomnia, and severe
muscle spasms. On the seventeenth day of her strike, she remains
defiant, refusing both her regular medication and sedatives. As a
result, she is enduring chronic pain, and the recurrence of her nervous
attacks is a looming threat. Ahmadi had previously suffered a nervous
breakdown during a security forces' raid on the women's ward of Evin
Prison and was subsequently transferred to the hospital. After an
assessment, doctors expressed concerns that she may have epilepsy and
emphasized that she requires specialized medical care outside the
prison. Sarvnaz Ahmadi was arrested during the nationwide protests in
2022. After a period of detention, she was released on bail but was
re-arrested on April 27, 2023, at the home of teacher activist Mohammad
Habibi. Branch 15 of the Tehran Revolutionary Court sentenced her to
five years in prison for <collusion and assembly against national
security> and an additional year for <propaganda against the regime.>
This sentence was later reduced to 3.5 years by Branch 36 of the Tehran
Court of Appeals. Sarvnaz Ahmadi holds a master's degree in social work
from the University of Rehabilitation Sciences in Velenjak. The
imprisonment of individuals with serious illnesses is a blatant
violation of the fundamental principles of the Universal Declaration of
Human Rights. This practice not only contradicts basic human and moral
values but also violates several articles of this Declaration.>>

prison.
Incarcerated in Mashhad, Fatemeh Sepehari is suffering from a high and
irregular heart rate, along with chest and back pain, according to her
brother. Sepehari's last cardiac examination was on May 1, with
recommendations for quarterly check-ups. However, prison authorities
have denied her requests for medical leave despite her worsening
condition. "Currently, my sister is suffering from high and irregular
heartbeat as well as chest and back pain," Asghar Sepehari said. He said
an orthopedic specialist had prescribed ten physiotherapy sessions for
his sister's hands, but she has only been allowed to attend two sessions
so far. The outspoken pro-democracy advocate was arrested in Mashhad on
September 12, 2022, following protests sparked by the death of Mahsa
Amini in police custody. She was subsequently sentenced to 18 years and
six months in prison by the Revolutionary Court in Mashhad on charges
including <assembly and collusion,> <insulting (Supreme Leader) Khamenei,>
and <propaganda activity against the Islamic Republic.> She is one of
the 14 signatories of a 2019 statement calling for Iran's Supreme Leader
Ali Khamenei's resignation, the abolition of the Islamic Republic, and

<<Outbreak of a Virus in Women's Ward of Evin Prison and Lack of Medical
Care
According to reports, a flu-like virus has recently spread in the
women's ward of Evin Prison, infecting a large number of female
political prisoners. The condition of some prisoners with underlying
diseases has been reported as critical. These include political
prisoners Marzieh Farsi, who suffers from cancer, and Fatemeh Ziaii, who
has multiple sclerosis. Prison guards have not only failed to provide
any medical care to combat this disease but have also failed to provide
facilities to quarantine infected patients. The Women's Committee of the
National Council of Resistance of Iran calls on international human
rights and women’s rights advocates to take urgent action to release
female political prisoners, especially the sick, and to provide them
with medical access and treatment.

Outbreak in Evin Women's Ward Raises Alarms
An unidentified flu-like illness has rapidly spread among female
prisoners in the women's ward of Evin Prison, affecting a significant
number of inmates. The symptoms of the illness include severe coughing
with phlegm, headaches, body aches, fatigue, drowsiness, and weakness.
The situation has become so dire that many prisoners have had to receive
intravenous fluids.
Inadequate Medical Care and Quarantine Facilities
Despite the seriousness of the outbreak, the female prisoners are
reportedly being denied adequate medical care and access to proper
quarantine facilities to isolate those who are sick. This is especially
concerning given that some of the affected prisoners are elderly,
including political prisoners Parvin Mirasan and Raheleh Rahemipour,
both of whom are over 70 years old.
High-Risk Prisoners
The situation is further exacerbated by the presence of several
high-risk prisoners who suffer from pre-existing conditions. These
include serious illnesses such as cancer, multiple sclerosis (MS), and
cardiovascular diseases, making them particularly vulnerable to
complications from the flu-like illness.
Some of the most at-risk inmates include:
Raheleh Rahemipour (71 years old): A political prisoner with a brain
tumor and other serious health issues.
Marzieh Farsi: Another political prisoner who has previously battled
cancer and continues to suffer from its effects.
Houri (Fatemeh) Ziaii: A political prisoner with multiple sclerosis and
other infectious diseases.

Human Rights Violations in Evin Prison
The failure to provide adequate medical care to sick prisoners and the
lack of proper quarantine measures constitute clear violations of the
fundamental principles of the Universal Declaration of Human Rights.
This treatment not only contravenes human dignity and ethical standards
but also breaches several specific articles of the declaration,
including Article 3 on the right to life, Article 5 on prohibition of
torture and inhuman treatment, and Article 7, the principle of equality.
The ongoing health crisis in Evin Prison's women’s ward underscores the
urgent need for international attention and intervention to protect the

severe health problems. Masoud Jamaei, a political detainee held in
Sheiban Prison in Ahvaz for over 13 months without a clear resolution to
his case, is reportedly suffering from severe health issues and has been
denied adequate medical care. According to the HRANA human rights
website, Jamaei is battling stomach and liver cancer, as well as high
blood pressure.
Despite his critical condition, Jamaei has been deprived of specialized
medical care and has not been transferred to any medical facilities for
treatment. Jamaei was previously charged by the prosecutor's office with
<corruption on Earth,> <membership in anti-Islamic Republic groups,> and
<assembly and collusion to disrupt national security.> However, despite
these serious allegations, no court has convened to hear his case.
Jamaei, 47, a resident of Ahvaz, was arrested by security forces from

<<Parvin Mirasan: Medical Care Denied to 67-Year-Old Prisoner
Parvin Mirasan, a 67-year-old political prisoner, is currently serving a
four-year sentence in Evin Prison. Despite needing specialized medical
attention, she was recently taken to a hospital but was returned to
prison shortly after, without completing the necessary treatment. Parvin
Mirasan suffers from multiple ailments, including diabetes, Parkinson's
disease, and joint issues, and she also occasionally experiences bowel
adhesions. She endures severe knee pain, with her left knee frequently
swelling, making it difficult for her to walk. Two weeks ago, after
months of being denied treatment, she was finally taken to the hospital,
where doctors advised that her left knee requires surgery. Nevertheless,
she was returned to prison without the necessary medical intervention,
only receiving pain relievers. Ms. Mirasan sometimes experiences severe
forgetfulness and prolonged silences, staring into space; at other
times, she speaks suddenly and at length. Despite her multiple illnesses
and neurological issues, she has still not been allowed a referral to
the medical examiner to assess her inability to withstand prison
conditions. Parvin Mirasan was arrested on November 4, 2022, during
nationwide protests by intelligence agents in Shiraz and was initially
held in Adelabad Prison. In February 2023, she was transferred to Tehran
and, on March 18, 2023, after her interrogation process ended, was moved
to the women's ward of Evin Prison. After 17 months of uncertainty, she
was sentenced in April this year by Branch 26 of the Tehran
Revolutionary Court to four years in prison on charges of assembly,
collusion, and propaganda against the state. Born in 1957, Parvin
Mirasan lost both of her sons when they were young and now has only one
daughter, who lives abroad.>>

Iranian protest singer Saman Yasin has been released on medical
furlough, his lawyer announced on Wednesday. Yasin's lawyer, Ramin
Safarnia, confirmed on social media that his client was granted
temporary release from Ghezelhesar Prison on bail. "Today, my client,
Saman Yasin, after enduring around 26 months in prison, was released on
medical leave following the recommendation of a medical commission and
with the judiciary's approval, after posting bail of 4 billion tomans
[$59,000]," Safarnia said. Legal expert Ali Mojtahedzadeh suggested that
Yasin's temporary release could lead to permanent freedom, noting that
his case falls under the 2022 amnesty order by Supreme Leader Ali
Khamenei. Yasin, a vocal critic of Iran's clerical establishment, was
arrested on October 2, 2022, and reportedly tortured during his
detention. He was sentenced to death on October 29 for <waging war
against God.> However, the Supreme Court later overturned the sentence.
In a letter earlier this year, Yasin highlighted his ongoing struggles
with mental and physical health issues, stating, "I continue to suffer,
yet incompetent officials cannot even facilitate my access to medical
care. Please inform me of the crime I have committed. If I am found
guilty, then imprison me; if not, then acquit me," he wrote.>>

Treatment
Afshin Baymani, a political prisoner incarcerated in Qezelhesar prison,
was rushed to a hospital outside the prison on Thursday night due to
symptoms of a heart attack. However, he was returned to prison without
receiving adequate medical treatment, raising serious concerns about his
health. According to IranWire sources, Baymani, who has been sentenced
to life imprisonment and suffers from coronary artery disease, has long
been denied proper medical care and access to specialized treatment
centers due to obstruction by prison officials. Judicial authorities
have also opposed his release. Baymani fainted and collapsed in prison
after experiencing a severe drop in blood pressure. The impact caused
him to fracture his head and suffer from bleeding after hitting a wall.
After examining him, the prison medical staff emphasized the need to
send him to the hospital for an angiography. Baymani requested medical
leave to receive treatment under the supervision of his specialist
doctor, but this request was denied by prison officials. In January,
Baymani was taken to Rajaei Shahr Hospital in Karaj due to his
deteriorating health but was returned to prison the following day
without receiving specialized medical care. He had previously been
granted medical leave to undergo open-heart surgery but was re-arrested
in Isfahan and transferred back to Qazalhasar Prison. Baymani, who is
married and has two children, was arrested in August 2000 and shortly
after that was sentenced to death by the Tehran Revolutionary Court on
charges of <waging war against God.> After six years, this sentence was
commuted to life imprisonment, and he has since been serving his
sentence in various prisons.
Many political prisoners in Iran are systematically and deliberately
denied access to proper medical care.
These denials are often used as a means of political repression,
intended to intimidate and suppress opposition voices.>>
