<<Empty Pockets and Emptier Plates: Poverty Grips Iran
The latest report from Iran's Parliamentary Research Center paints a
devastating picture of nutritional distress among Iranians. With 30 per
cent of the population living below the poverty line, the country faces
a health catastrophe that extends far beyond economic statistics - it’s
a matter of basic human survival. The report, which provides extensive
data on Iran's economy in 2023, indicates no improvement in the poverty
rate, which encompasses a third of the population. Poverty affects
citizens' lives in various ways, but its most serious and critical
consequence is its impact on the physical and mental health of those
affected. In 2022, the parliament reported that half of Iran's
population consumed less than the standard 2,100 daily calories. Across
all income groups, calorie intake dropped compared to 2021, with
middle-income groups experiencing the most significant reductions. Last
year, Hadi Mousavi, the former Director-General of Social Welfare
Studies at the Ministry of Labor, warned that 57 per cent of Iranians
suffer from malnutrition, failing to meet the daily 2,100-calorie
requirement. The decline is not due to changing dietary habits but
rather to economic struggles. Yasaman Hashemi, a nutritionist,
identifies various forms of malnutrition as a direct result of poverty.
Speaking to IranWire, she explained: "We typically see two types of
malnutrition. One results in extreme thinness, nutrient deficiencies,
protein shortages, weakened immune systems, and increased vulnerability
to infectious diseases. The other type leads to abdominal obesity due to
reliance on starchy foods, which are less nutritious. Such diets often
cause chronic inflammation, cardiovascular diseases, hypertension, fatty
liver, and type 2 diabetes, all of which are prevalent in society."
Meanwhile, the Secretary of the Association of Animal Product Importers
said that annual red meat consumption in Iran has dropped by one-third,
falling below 600,000 tons per year. The UN Food and Agriculture
Organization estimates that in 2023, meat consumption in Iran declined
further, with per capita meat intake one-third lower than the global
average. Dairy consumption has also significantly decreased due to
reduced purchasing power. Hashemi highlighted the health consequences of
this trend: lower dairy intake increases the risk of dental problems and
osteoporosis. She added that low-income families often cannot afford
healthy fats such as fish or nuts like walnuts. The lack of omega-3-rich
foods contributes to neurological issues, including depression. Due to
the high cost of chicken, meat, and fish, access to these protein
sources has become extremely limited. A lack of sufficient protein
negatively impacts children's growth, weakens the immune systems of the
elderly, and delays recovery for those who fall ill. Poverty, coupled
with malnutrition, also impacts the costs people must allocate for their
health. Hamshahri newspaper reported that in the past 20 years, tooth
decay among Iranians has tripled. The head of the Iranian Dental
Association has also spoken about a continued decline in dental visits
due to widespread financial difficulties and poverty. According to
government statistics, dental health in Iran is in poor condition. On
average, Iranian six-year-olds have over five decayed teeth, adults aged
30 to 40 - at the peak of their social and professional activity - have
lost 12 to 13 teeth, and over 55 percent of people aged 65 and above
have no teeth. Sahar Motallebi, a physician and researcher in global
health and population studies, told IranWire that in recent years, the
cost of medicine, medical treatment, and dental care has been eliminated
from the budgets of households living below the poverty line.
Highlighting the widespread malnutrition caused by poverty and its
severe physical and mental health consequences, she pointed to the
prevalence of osteoporosis among Iranian women. "In Iran, 47 percent of
women over 50 suffer from osteoporosis, and calcium deficiency is
widespread. Poverty limits access to calcium- and vitamin D-rich foods,
which are essential for bone health," she said. "The lack of these
nutrients increases the risk of serious fractures in old age. The
inability to afford dietary supplements or diagnostic bone density tests
exacerbates these risks," she added. A doctor in Iran said poverty
affected medical treatment, adding that due to shortages of certain
medications, including chemotherapy drugs, patients are forced to pay
exorbitant prices to procure them from abroad or on the black market.
"If a patient cannot afford these medications, they don't even try to
obtain them, and unfortunately, cancer patients often die sooner as a
result." The doctor added that some patients arrive at emergency rooms
with critically low levels of consciousness due to blood sugar and
potassium imbalances caused by poor nutrition and the inability to
access essential medicines. Motallebi also explained the consequences of
untreated diseases. Chronic conditions like diabetes and hypertension
are more prevalent among low-income people due to their lack of access
to adequate healthcare. For diabetics, the lack of regular care and
treatment can result in diabetic ulcers and even amputations.
Uncontrolled diabetes may also lead to diabetic retinopathy, which can
ultimately cause blindness. The report by the Parliamentary Research
Center reveals that even in years without negative economic growth, the
poverty rate has remained unchanged. This indicates that economic growth
has not been inclusive, leaving low-income groups unable to benefit from
it. High reliance on oil for economic growth and near-zero growth in the
agricultural sector - where a significant portion of impoverished
employment is concentrated - are among the signs of this lack of
inclusivity. Saba Alaleh, a psychologist, pointed to the psychological
impact of malnutrition, such as increased depression and anxiety. She
explained that one of poverty's psychological effects is the experience
of deprivation and prolonged, repeated failures. According to her,
poverty, discrimination, and inequality directly affect mental health
and cognitive functions, including decision-making, problem-solving, and
future planning. Poverty also significantly impacts verbal and
communication skills. "One of poverty's most critical consequences is
that people either develop suicidal thoughts or attempt suicide," she
said. Studies have also shown that food insecurity caused by poverty
affects over 70 per cent of the population in some provinces. Hossein
Raghfar, an economist and professor at Al-Zahra University in Tehran,
cited data from the National Supreme Council for Health and Food
Security, revealing that 58 per cent of families in southern Khuzestan
province face food insecurity. The figure rises to 70 percent in Sistan
and Baluchistan and 74 per cent in Kerman. He said that global per
capita meat consumption averages 30 kilograms, while in Iran, it has
fallen to five kilograms - and even this is unevenly distributed among
low-income groups. Alaleh explained that impoverished people often face
social stigma and extensive deprivation, leading to social isolation.
This isolation and exclusion endanger their mental health, making them
vulnerable to various psychological disorders. The Parliamentary
Research Center's report on persistent poverty, class disparities, and
inequality in Iran comes as the looming threat of conflict with Israel
adds to societal strain.
Psychologists warn that war severely harms mental and social well-being.
However, they also view poverty, especially after war, as a force that
can break a society's psychological foundation and lead to its
